What is Pilonidal Disease?
Pilonidal disease occurs when a cyst or abscess forms near the tailbone, often due to trapped hair, dirt, and debris in between the upper buttock fold. This causes an inflammatory cavity and abscess which can subsequently discharge out via various pits and sinuses.

Symptoms of Pilonidal Disease.
Some common symptoms of pilonidal disease include:
- Pain near the tailbone, especially when sitting
- Swelling or redness around the area
- Drainage of pus or blood from the cyst, which can be persistent

How Pilonidal Disease Surgery Can Help
Although not life-threatening, the management of pilonidal disease is complex. During the ‘acute phase’, the initial redness and throbbing might at least partially respond to antibiotics. However, if there is a sizable lump (abscess), surgery is required. An ‘incision and drainage’ is performed, to drain the pus and relieve the pain.
After this, the natural history is variable. Some cases are lucky enough to completely settle down and do not recur. Others, however, experience recurrent infections and or persistent discharge. In these cases, a second more extensive surgery is required. The key is to remove not only the initial abscess (which is the ‘tip of the iceberg’), but the entire underlying inflammatory cavity and associated pits further down. This would leave a significant defect, and so some form of ‘plastic surgery to your behind’ is required to move healthy tissue to close the defect. This is obtained from the neighbouring buttock, which has tissue to spare.
This surgery is safe and not overly painful, but does require general anaesthesia, an overnight hospital stay, and 3-6 weeks of healing (not placing direct weight on the area, dressings, reviews). However, when the disease and discharge has begun to impact your qualify of life, it becomes warranted. This gives the best chance of resolving the problem.
